Shift- Days (8-hour shifts) + every 3rd weekend; first to float
Contract: 13 weeks 
Pay Term- Weekly (By Every Friday)

Job Description:

 

Unit & Responsibilities
  • Unit: Care Coordination (Case Management)
  • Unit Type: Case Management (Utilization Review)
  • Division: Nursing
  • Care Setting: Acute Care
  • Patient Ratio: 1:20

     

  • Qualifications
  • Experience: Minimum 3 years hospital experience with acute care coordination; must have prior CA Dignity/CommonSpirit acute care experience
  • Education: BSN not required
  • Licenses / Certifications: RN license required; BLS required
  • Orientation: 12 hours required, plus floor-specific training (4-day webinar + 2-day preceptor)
  • References: 2 supervisor references required (1 within 12 months, 1 within 3 years)

     

  • Responsibilities:
  • Perform acute care coordination, discharge planning, and utilization review
  • Float to cover all adult areas of the hospital (women’s health experience required to float there)
  • Complete charting in Cerner, Curaspan, Midas, InterCall, and Microsoft Office
  • Collaborate with interdisciplinary teams to ensure high-quality patient care
  • Maintain excellent attendance, adaptability, and professional demeanor
