Social Worker (LMSW) - Charles County MD

Currently seeking a Social Worker, LMSW for a non-profit hospice provider in Charles County, MD. Organization is well established, with a great reputation for quality of care in the state of Maryland. Position offers great work life balance and the opportunity to spend one on one patient time!

Schedule: Monday - Friday 8:00AM - 4:30PM. No on-call or weekend requirements, but opportunity to pick up extra hours if desired for additional income.

Benefits and Compensation for the Social Worker, LMSW:

• Highly competitive hourly rates ranging from $70,000.00 - $80,000.00 per year depending on years of experience or qualifications

Responsibilities of the Social Worker, LMSW:

• Create and implement individualized plans of care for end of life patients residing in private homes and facilities across Charles County, MD
• Provide emotional support and counseling for patients
• Collaborate with interdisciplinary team (physician/provider, RN case manager, chaplain, etc) to provide the highest level quality of care.

Requirements of the Social Worker, LMSW:

• LMSW, LGSW or LCSW-C Licensure is required for this role
• Minimum of 1-2 years of previous clinical experience working with the geriatric patient population ideally in long term care, hospice, home health, acute care or related setting
