Social Worker
Schedule: 9:00 AM – 5:00 PM
Weekly Hours: 35
Job Purpose
The Social Worker provides essential services to participants, advocating, assessing, assisting, collaborating, educating, evaluating, implementing, and planning for needs across psychological, psychosocial, financial, environmental, and interpersonal areas. Services may take place in participants’ homes, community sites, or care facilities.

This role serves as a bridge between participants, their families/representatives, the interdisciplinary care team, and community agencies to ensure comprehensive support and high satisfaction. Responsibilities include psychosocial assessments, care planning, crisis intervention, adjustment support, and coordination of community services.

Key Responsibilities

• Provide social work services to participants and families to promote understanding of care plans and address barriers.
• Educate participants and families about community resources and assist with referrals, coordination, and paperwork.
• Coordinate care and communication during/post hospitalizations or facility stays.
• Deliver consultation and education on treatment options, including palliative and end-of-life care.
• Conduct biopsychosocial and cognitive/emotional assessments; assist with advance directives.
• Ensure care plan goals and interventions are participant-centered and applied.
• Maintain accurate and timely documentation in the EMR (within 24–72 hours).
• Use case management and clinical skills to address social, financial, and psychological challenges.
• Facilitate psycho-educational presentations at day centers and promote the social work role.
• Participate actively in care team meetings, contributing social work interventions and recommendations.
• Engage in continuing education, peer supervision, and professional development.
• Represent the Social Work department at meetings and community events.
• Perform other duties as assigned.

Qualifications
Education:

• Master’s degree in Social Work (MSW) from an accredited institution.

Experience:

• At least one (1) year in a healthcare setting.
• At least one (1) year working with frail or elderly populations (or appropriate training will be provided).
• Home care/fieldwork experience preferred.
• Regular access to a car for field visits preferred.

Licensure:

• Active New York State LMSW or LCSW license required.

Additional Requirements:

• Authorized to practice in New York State.
• Medically cleared and up-to-date on immunizations.

Physical Requirements

• Standing: up to 6 hours/day
• Sitting: up to 6–8 consecutive hours/day
• Lifting/Pushing/Pulling: up to 50 lbs (including safe patient handling)
• Ability to bend, squat, climb stairs, and walk between sites/locations
• Fine motor skills for equipment use and documentation
• Visual and auditory ability to assess patients and communicate effectively
• Strong cognitive and decision-making abilities
