The HCBS (Home and Community Based Services) Coordinator is responsible for overseeing and coordinating services for individuals enrolled in the HCBS program. The Coordinator ensures that clients receive personalized, quality care and support based on their Individualized Service Plans (ISPs). This role involves managing DSPs (Direct Support Professionals), collaborating with care managers, and ensuring compliance with state and federal regulations.

Key Responsibilities:

  • Client Intake and Assessment:
    Conduct initial assessments and assist clients and families through the intake process, ensuring all necessary documentation and eligibility criteria are met for HCBS services.
  • Service Plan Development:
    Collaborate with care managers, families, and other stakeholders to develop and implement Individualized Service Plans (ISPs) that meet each clients specific needs and goals.
  • Staff Supervision and Training:
    Supervise Direct Support Professionals (DSPs), provide guidance and support, and ensure they receive appropriate training on client-specific needs and HCBS program regulations.
  • Coordination of Services:
    Schedule and monitor the delivery of services, ensuring that clients receive timely and effective support in areas such as personal care, daily living activities, and community participation.
  • Compliance and Documentation:
    Ensure that all client records, progress notes, and service documentation are maintained accurately and in compliance with HCBS program regulations. Oversee audits and reporting processes as required by state and federal guidelines.
  • Family and Client Liaison:
    Serve as the main point of contact for clients and their families, providing ongoing communication about the status of services, addressing any concerns, and making adjustments to ISPs as needed.
  • Quality Assurance:
    Monitor and evaluate the quality of care being provided, conducting regular reviews of service delivery and making improvements where necessary.
  • Collaboration with Care Managers and Other Providers:
    Work closely with care management teams, healthcare professionals, and other service providers to coordinate holistic support and ensure seamless care for each client.
  • Crisis Management:
    Respond promptly to any emergencies or crises, providing guidance and support to DSPs and clients, and ensuring that incidents are documented and reported according to protocol.

Qualifications:

  • Bachelors degree in social work, human services, healthcare administration, or a related field (preferred but not required with relevant experience).
  • Minimum of 1 year of experience in the human services field, with a focus on working with individuals with disabilities or HCBS programs.
  • Strong knowledge of HCBS program guidelines, state and federal regulations, and person-centered care principles.
  • Excellent organizational, communication, and interpersonal skills.
  • Leadership experience with the ability to supervise and support staff.
  • Proficient in managing documentation and client records in compliance with regulatory requirements.
  • Ability to handle crises and challenging situations with patience, professionalism, and composure.
  • Proficiency with office software (e.g., Microsoft Office) and ability to learn relevant case management systems.
