PURPOSE OF POSITION:

The Caring Transitions Team is comprised of Social Workers, Peer and Family Advocates, a Transitions Coordinator, and a Medical Director, who will support children, youth, and their families who present at H+H hospitals in Bronx following a suicide attempt or serious suicidal behavior, from point of entry until 90 days post-discharge.

The Caring Transitions Team will engage with youth and families upon presentation to the ED or direct admission.If interested in participating, the team will follow them throughout the hospital course, participating in aftercare planning.Upon discharge the team ensures that youth are connected to outpatient treatment, provides caregivers with tools to understand and identify risk factors and warning signs of suicide, assists the youth in developing life-affirming goals, and develops the capacity of other trusted adults and community supports in the lives of the youth and families they serve. In certain cases, the team will provide bridging care/treatment until the youth is connected to outpatient services.

SUMMARY OF DUTIES AND RESPONSIBILITIES:

The Community Liaison Worker is an integral part of this interdisciplinary team.Under supervision, the candidate for the position will work with a patient and their families performing patient-centered, community health work in close collaboration with the Caring Transitions Team.They will support the patient and family with follow-up and transition services.

The Community Liaison Worker participates in community outreach activities within the communities of NYC, engaging patients and providing resources and assistance needed to address issues preventing them from staying healthy.They provide direct services to families in emergency, inpatient, outpatient, home, and specialized settings. Works in close collaboration with staff, consumers, family, and community providers in planning and implementing treatment plans, and supporting and navigating the system of care. They motivate and support patients, parents, and caretakers to achieve their family health goals through facilitating connections to primary and specialty care clinics, community and parenting services, mental health support, and/or pre-natal care, creating health goals.

Patient and family support, advocacy, system linkage, and access to care.

GENERAL TASKS AND RESPONSIBILITIES WILL INCLUDE:

Maintains awareness of community-based resources.

Outreaches patients and families post-visit.

Conducts community outreach.

Engages patients, parents and/or caretakers, and/or community providers, as applicable, and provides resources and assistance needed to address issues preventing them from staying healthy.

Assists in the preparation of information and educational materials.

Keeps records related to programs, activities, etc.

Makes written and oral reports, as required.

Participates in the planning, organization, and implementation of events involving community participation.

Works as a member of an interdisciplinary team assisting in providing supportive therapy and making referrals to primary care providers, specialty care providers, community services and/or mental health services, medical appointments, and filling prescriptions.

Join patients during appointments and hospital stays as needed; communicate with their care team to help coordinate care.

Assess the ability of patients to manage their illness and work care team to connect them to appropriate education and monitoring programs.

Works with patients and families to assist them in completing forms; completes other relevant reports and forms and performs related clerical work.

Recruit eligible patients and conduct clinical or community-based assessments and interviews to identify their health goals and needs.

Identify barriers to carrying out plans, make referrals as appropriate, and communicate on these issues with the team.

Assess the ability of patients to manage their illness and work care team to connect them to appropriate education and monitoring programs.

Document each patient encounter in the electronic medical record (EPIC) and use other electronic systems (including but not limited to NowPow) in accordance with established policies and procedures.

Conduct home and community-based visits to assess patient needs specific to the home environment.

Report regularly to the Supervisor for support and feedback; participate in program improvement efforts.

Other assignments as assigned.

Minimum Qualifications
1. Four years of full-time experience in counseling, community work or community health activities in a government agency or community organization engaged in providing community services to the public, assisting members of the community in obtaining community services or maintaining liaison with schools, community organizations or other government agencies for the purpose of providing assistance and obtaining participation and support for implementation of community or public service programs; or,
2. Education and/or experience equivalent to (1 above).Study at an accredited college in Sociology, Psychology or other behavioral sciences may be substituted on a year-for-year basis for the experience described in (1 above).However, all persons must have at least one year of full-time experience as described in (1 above).
